nope..i almost tripped while i was on the treadmil though..that would be very embarrasing though if i actually hurt myself at the gym...
Not at the Gym, per se... but in my Gym room at home, yes.
The whole reason I have a Power Cage is so that I can self-spot on low-rep near-100% 1RM lifts; and surprisingly I haven't been injured doing these yet -- probably because I'm actually very aware and very careful during these lifts.
I injure myself when I'm being a dumbass though, and just being careless.
A while ago, I used to laugh at people who used Swiss Balls, until I realized how great they were in developing Core strength. I tried out a Swiss Ball Curl-Up with light weights, I liked it, and so I got up, and got what I figured my usual weight stack for Crunches should be. So, with my back sweaty and no towel on the ball; and nothing holding the ball steady as I positioned myself, I lay down on the Swiss Ball, hoisted about 150 pounds onto my chest to do hyperextended Curl-Ups...
...and the Swiss Ball shot out from under me.
The ball went in one direction, my feet went in the other, my head dropped two feet straight to the ground below, and 150 pounds of Olympic Iron Plates came crashing down my jaw and face.
Luckily, I'm used to taking massive trauma to the head on a twice-a-week basis.
So I ended up laughing my ass off; both thankful that nobody was there to watch that happen to me; and wishing that someone had recorded that on video because it would have been hilarious to watch. Lesson -- how NOT to use a Swiss Ball. Lesson -- don't be overconfident, you cocky ass (Iron plates landing on head can hurt sometimes!). Injuries, not funny. But at the same time, LMAO.

YES. yesterday. basketball. sprained ankle. then some 260lbs genius decided to step on my sprained ankle (albeit accidentally). and now, my ankle is the size of an apple.
I injured myself pretty frequently at GYMnastics, but at the gym, no. Once I fell off a treadmill, but I was at school and we were going at pretty much top speed to see who'd fall off first (fyi, I won. It was painful and fully worth it. I won a muffin).
I don't even need a gym, I can injure myself getting out of bed in the morning. :)
No but does working out too hard to where you hurt count?
Only through performance and not actually in the gym. take it easy and learn from others. Be open, don't be shy.

oh i thought you meant during "gym class". i was once hit by a badminton racket during gym class.

yeah lol. it was a total fail LOL. >__> i totally embarrassed myself lol. it was horribly embarrassing. let's just say treadmill. scraped my knees and all.

But then I knew a girl like that once. We went to the gym and she got on the treadmill next to mine. Instead of slowly warming up she started running right away. Since it was winter she was wearing a sweater on top of a tank top. She decided to take it off but in that sexy slow motion way in the movies. Needless to say it got stuck on her arms and she couldn't see, fell and face plant like the vid. Then when she got on again, she jumped on it instead of stepping onto the sides and WHAM went down again face plant!
